Claim Forms

In order for Therma-Tru to reimburse you for advertising/promotional purposes, you must fill out the proper documentation. Authorized distributors may submit claims online, click here. Others may submit their claim forms through a Therma-Tru distributor.
 
Co-op Advertising Claim Form
All authorized full-line/full-system distributors of Therma-Tru are eligible to participate in our Co-op Program. To receive proper reimbursement, use the Co-op Advertising Claim Form for the following items:

Model Home Credit Form
Only authorized Certified Distributors of Therma-Tru are eligible to participate in our Model Home Program. Authorized distributors must remit completed credit forms approved by a Therma-Tru Territory Manager, an invoice and a photo of the model home to the Co-op administrator at Therma-Tru. The Co-op Model Home Credit Form will also be used to calculate reimbursement for Design Center display doors, which also require a Therma-Tru Territory Manager's approval.

Showroom Credit Form
A maximum of six (total) Classic-Craft, Fiber-Classic or Smooth-Star fiberglass door systems are eligible for Co-op reimbursement. Six different doors must be displayed to be eligible for Co-op. Exception is when a double door unit is installed as the entry way. Use the Co-op Showroom Credit Form to calculate the reimbursement. The signature of the appropriate Therma-Tru Territory Manager, invoice and photographs of all doors must be submitted with the claim form to qualify for reimbursement.
